Crestview Area Shelter for the Homeless Public Notice and Privacy Notice

Crestview Area Shelter for the Homeless receives funding from Homeless Housing Alliance (HHA) to provide services for homeless and near homeless individuals and their families. A requirement of this funding is that the agency participates in HMIS, the Homeless Management Information system for the Program Management System of the Southeast (PromisSE), which collects basic information about clients receiving services from this Agency. This requirement was enacted in order to get a more accurate count of individuals and families who are homeless and to identify the need for different services. We only collect information that we consider to be appropriate. The collection and use of all personal information is guided by strict standards of confidentiality.

A copy of our Privacy Notice describing our privacy practice is available to all consumers upon request. You do have the ability to share your personal information with other area agencies that participate in the network by completing a Release of Information form or verbally consenting to the Release of Information. This will allow those agencies to work in a cooperative manner to provide you with efficient and effective services. The collection and use of all personal information is guided by strict standards of confidentiality. A copy of our Privacy Notice can also be found in our HMIS Policies and Procedures.

Since retiring from teaching, Ann has dreamed of lending a helping hand to those in need.

House Director
Carrie Johnson

Between 10 to 25 people may reside in the shelter at any given time.  Carrie works with a diverse group of individuals within the shelter. Despite the challenges of guiding them through shelter life, she manages it with remarkable ease and grace.

Case Manager
Jim Gaylord,
Michelle Adams

Jim and Michelle are dedicated to assisting the homeless with their various needs. They provide help with obtaining IDs, birth certificates, and bus tickets to secure a place to live. There are numerous other services offered in this office, so please come in to see how we can assist you.

Outreach Team Dave

This group excels in providing assistance where it's needed, from welcoming individuals as they arrive to fulfilling their requirements. They adeptly handle everything from tallying donations to sorting and storing them in the back room.

Hope Starts Here Angela Jones

Our quaint thrift store is situated at 192 Main St., Crestview, FL. Angela has done an incredible job setting it up and maintaining it for your visit. We receive new items every day.

Disabilities Advocate
Karo and Melody

If you require assistance navigating the disability system, this team member is an excellent resource. Together with her team, they have aided numerous individuals in obtaining disability benefits, enabling them to afford their own housing.

Transportation Specialist Jerry

We have a driver who occasionally picks up clients and brings them to their doctor's appointments.

Office Manager

Renee is the linchpin of the office, maintaining everyone's focus with her exceptional organizational abilities.

Assistant Director Neal Johnson

Neil is a Marine Veteran, and we are pleased to welcome him to our team. He is collaborating closely with Ann Sprague to learn the operational aspects of the office at the shelter.
